This assessment measures your brain’s capacity to handle and recover from neurologic stressors such as surgery and anesthesia. It takes approximately 16–22 minutes to complete. Your answers are confidential and will be used to generate your personalized results.

Who We Are

 

Beyond Brain Health (beyondbrainhealth.com) provides the Neurologic Stress & Recovery Index™ (NSRI™), a pre-surgical brain optimization assessment developed by Sandra Bargeron, PA-C, CAA. This policy explains what data we collect when you complete the NSRI™ assessment and how we use it.

 

What We Collect

 

When you complete the NSRI™ assessment, we collect the information you provide directly, including:

 
  • Basic demographics (age range, biological sex, education level)
  • Health and medical history responses (medications, sleep, lifestyle, surgical history)
  • Planned surgical procedure and timing
  • Contact information (name and email address) used to deliver your report
 

We do not collect Social Security numbers, financial information, or government-issued ID.

 

How We Use Your Information

 

Your responses are used to:

 
  • Calculate your NSRI™ score and generate your personalized report
  • Deliver your PDF report to the email address you provided
  • Improve the accuracy and quality of the assessment over time
  • Contribute to de-identified research datasets that may be licensed or sold to third parties for scientific, academic, or health research purposes only
 

We do not sell your name, email address, or any information that could be used to identify you. Any data shared or sold for research purposes is de-identified and aggregated prior to release. We do not use your information for advertising or marketing purposes.

 

Research Data & De-Identification

 

De-identified data collected through the NSRI™ assessment may be used in research collaborations or sold to third-party research organizations. Before any such use, all direct identifiers — including name, email address, and any other information that could reasonably be used to identify you — are removed. Data released to third parties contains only anonymized health and demographic response patterns. No individual responses are traceable back to you.

 

Who Processes Your Data

 

Your assessment responses are processed through secure, HIPAA-aware infrastructure. We use the following third-party services to operate the platform:

 
  • LimeSurvey — LimeSurvey
  • Anthropic (Claude API) — personalized report text generation (de-identified scoring data only; name and email are never transmitted)
  • Email delivery service — report distribution
 

All services are contractually bound to handle your data with confidentiality.

 

Data Retention

 

Your assessment responses are retained for up to 12 months to allow for follow-up assessments and score comparisons. You may request deletion of your data at any time by contacting us at [email protected].

 

Medical Disclaimer

 

The NSRI™ is not a diagnostic tool and does not constitute medical advice. Your responses are not shared with your healthcare providers unless you choose to do so by sharing your report. We are not a covered HIPAA entity; however, we apply HIPAA-aligned data handling practices as a matter of best practice.

 

Your Rights

 

You have the right to:

 
  • Request a copy of the data we hold about you
  • Request correction or deletion of your data
  • Withdraw consent at any time (note: this does not affect data already processed)
